Medication name: Synjardy Empagliflozin 12.5 mg / Metformin 1000 mg Tablets × 60
Drug class: Combination antidiabetic agent (SGLT2 inhibitor + Biguanide)

Use:
This medication is used to control blood sugar levels in adults with type 2 diabetes mellitus, especially when a single medicine is not enough.
It combines two active ingredients empagliflozin and metformin to improve overall glucose control, reduce cardiovascular risk in diabetic patients with heart disease, and support kidney protection in long-term diabetes management.

How it works:

Empagliflozin, an SGLT2 inhibitor, lowers blood sugar by preventing glucose reabsorption in the kidneys, causing excess sugar to be eliminated through urine.

Metformin, a biguanide, works mainly by reducing glucose production in the liver and increasing the body’s sensitivity to insulin.
Together, they target different mechanisms to achieve more effective and sustained blood sugar reduction, while also promoting modest weight loss and lowering blood pressure.

Dosage and administration:
The usual adult dose is one tablet twice daily, taken with morning and evening meals to minimize stomach upset.
Swallow the tablet whole with water — do not crush or chew.
Your doctor may adjust the dose depending on your blood sugar levels and kidney function.
Continue taking the medication regularly and do not skip meals to prevent low blood sugar.

Side effects:
Synjardy is generally well tolerated.
Common mild side effects include increased urination, thirst, nausea, or mild stomach discomfort.
Genital yeast infections or urinary tract infections may occasionally occur due to increased sugar in the urine.
Rare but serious side effects include lactic acidosis (caused by metformin) and ketoacidosis (caused by empagliflozin).
Seek medical help immediately if you experience deep or rapid breathing, severe weakness, nausea, or abdominal pain.

Precautions:
Do not use if allergic to empagliflozin, metformin, or any of the ingredients.
Avoid use in patients with severe kidney disease, dehydration, heart failure, or liver problems.
Limit alcohol intake, as it increases the risk of lactic acidosis.
Not suitable for patients with type 1 diabetes or diabetic ketoacidosis.
Consult your doctor before use if you are pregnant, breastfeeding, or planning surgery or radiologic procedures with contrast dye.

Storage:
Store below 25 °C in a dry place away from direct sunlight and moisture.
Keep tablets in their original packaging until use.
Keep out of reach of children and do not use after the expiry date printed on the box.
