Mechanism of Action

• The exact mechanism is not fully understood.

• In schizophrenia: Antagonism of dopamine D₂ and serotonin 5-HT₂A receptors reduces psychotic symptoms.

• In bipolar or major depression: Effects may relate to binding to the norepinephrine transporter.

• Additional receptor actions:

  • H₁ blockade → somnolence

  • α₁-adrenergic blockade → orthostatic hypotension

  • M₁-muscarinic blockade → anticholinergic effects (dry mouth, constipation)

Therapeutic Indications

• Schizophrenia: hallucinations, delusions, confusion, suspiciousness.

• Bipolar mania: over-excitement, agitation, hyperactivity.

• Bipolar or major depression: sadness, guilt, low energy, poor sleep or appetite.

Method of Administration

• Take once daily by mouth.

• Swallow whole with water; do not split, crush, or chew.

• Take without food (at least one hour before a meal or at bedtime).

• Avoid grapefruit juice.

• Do not stop taking without medical advice.

• Follow the dose prescribed by your doctor.

• If an overdose occurs, seek medical attention immediately.

• If a dose is missed, take it when remembered unless near the next dose; do not double the dose.

Undesirable Side Effects

• Common: dizziness, headache, dry mouth, drowsiness, constipation, indigestion.

• Metabolic: increased blood sugar, cholesterol, triglycerides, weight gain.

• Cardiovascular: low blood pressure when standing, rapid heartbeat.

• Neurologic: tremor, stiffness, abnormal movements, restlessness.

• Endocrine: raised prolactin, thyroid changes.

• Other: blurred vision, fatigue, swelling, abnormal dreams, irritability, speech disturbance, fever, liver enzyme increase, reduced blood cells.

• Withdrawal: insomnia, nausea, vomiting, dizziness, irritability if stopped suddenly.
